Cygnus thinks that one of the outputs have already been added and therefore the file cannot be rebuilt.

This is quite common within a Splerge following a crash and is down to the Splerge being out of sync with the previous module.  Clearing down and re-running the previous module then rebuilding the Splerge will usually clear it (if huge file consider inserting a temp module to allow Splerge to regulate).
